Project The CVD Prevention Decathlon – a culturally tailored, highly personalised behaviour change programme that rewards people for becoming healthier and empowers people to improve their health
Description

The CVD Prevention Decathlon will provide structured education for those at risk of Cardiovascular Disease (CVD). Fusing structured learning and physical activity delivered by Xyla, gamification via the bespoke Sweatcoin app and personalised behaviour change videos via Citizen communications, the aim is to prevent the onset of CVD for local populations.

Project Making Young People’s Asthma Self-management Smarter (MYPASS)
Description

The Digital Health Passport is an asthma self-management app co-designed with young people and NHS specialists; aligned to clinical guidelines (NICE, BTS/SIGN, GINA) to improve asthma control. MYPASS will measure the impact of the platform on asthma attacks, unscheduled care and steroid use and evaluate three user engagement strategies (Nurse-led, Social Media and NHS Comms).

MYPASS will deliver:
Smarter asthma plans with signposting from symptom trackers.
Improved medication adherence - repeat prescriptions, medication reminders, inhaler videos.
Trigger avoidance - air quality alerts, allergy education.
Knowledge - seven core modules with regular reinforcement.
Improved patient activation - skills, knowledge and confidence to self-manage.
